SCOPE OF SERVICES
I AM Companion Home Care, LLC is a home care agency that provides both skilled and personal care services. Available 24hrs a day, 7 days a week.
NURSING
Includes but is not limited to:
- Provision of skilled nursing services
- Medication administration
- Assessment of needs
- Wound care
- Diabetic care
- Patient and family education
- Supervision of LPN’s and HHA’s
- Coordination of care
PHYSICAL THERAPY
Includes but is not limited to:
- Evaluate and establish treatment programs for patients requiring physical therapy
- Gait training
- Muscle strengthening
- Posture training
- Training on mobility aids
OCCUPATIONAL THERAPY
Includes but is not limited to:
- Evaluate and establish treatment programs for patients requiring occupational therapy
- Training related to self-care activities (feeding, dressing, bathing.)
SPEECH THERAPY
Includes but is not limited to:
- Provide assessment, treatment, and intervention for patients with speech and language disorders
- Post-CVA assessment and treatment
MEDICAL SOCIAL SERVICES
Includes but is not limited to:
- Evaluate current living situation including support network
- Connect patients and families to necessary community resources
- Participate in discharge planning, as necessary
HOME HEALTH AIDE
Includes but is not limited to:
- Taking a patient’s temperature, pulse and respiration
- Giving personal care including bed baths, sponge, tub or shower bath, shampoo, nail and skin care, oral hygiene, assisting with toileting and elimination, assisting with dressing
- Assistance with patient transfer and ambulation
- Assistance with self-administration of medications
- Patient positioning, observation, reporting and documentation of patient status and the care or services provided
- Maintenance of a clean and safe environment, light housekeeping
- Meal preparation
HOMEMAKER
Includes:
- Providing companionship and conversation
- Light housekeeping
- Household management
- Meal planning and preparation
- Errands, transportation and shopping
- Apparel selection
- Laundry, ironing, linen change
- Prepare sleeping area at night
- Assistance with bathing areas the patient cannot reach
- Fastening the patient’s clothing
- Combing hair
- Brushing dentures
- Shaving with an electric razor
- Providing stabilization to a patient while walking
